The Unique Student Identifier (USI) is a national government initiative for all students undertaking nationally recognised training from 1 January 2015.

If you are a new or continuing student undertaking nationally recognised training, you need a USI in order to receive your qualification or statement of attainment. If you do not have a USI, or do not provide your USI to us, you will not be able to receive your qualification or statement of attainment.

How do I apply for a USI?

Creating a USI will only take a few minutes and it is free. You can apply for your USI via the USI website at https://www.usi.gov.au/. You only need to create a USI once and it will stay with you for life.

In order to create your USI, you will need at least one form of ID such as an Australian Passport, Birth Certificate, Driver's Licence or Medicare Card.

If you have undertaken nationally recognised training since 1 January 2015, you may already have a USI. You only ever need one USI. Just like a Tax File Number, it is unique to you and it is yours for life.

I agree to attend all required training dates and if I am unable to, I will contact HITsa prior to the commencement of training to advise of my absence. 

I understand that if I do not attend training and do not contact HITsa prior to the commencement of training all fees paid will be forfeited and it is at the discretion of HITsa to reschedule any missed training dates.

I authorise HITsa to obtain, discuss, and release personal information regarding my academic history, academic transcript, certificates, class attendance, punctuality, identified barriers risking successful completion and training progress to any of my Employer, Employment Service Provider, Case Worker, Educational Institution I am or have previously been enrolled with, and / or any relevant State or Federal Government department or agency. 

I authorise HITsa trainers and staff who are directly involved with my training to share my personal information with each other to successfully support my learning.  The information that may be shared includes my name, the location of my training and my personal learning support needs.

I authorise HITsa  to share my personal information with trainers and assessors approved by Skills SA to interpret my Core Skills Profile for Adults (CSPA) report.  Information will be shared if testing indicates that I have not met the minimum ACSF Exit Level.  My personal information that may be shared includes my name, date of birth, contact details, USI, Up front Assessment of Need results, course applied for, CSPA results.

I understand that I may withdraw this authorisation at any time by submitting a request in writing to HITsa.

HITsa will arrange work placement with industry partners on behalf of students for Childhood Education and Individual Support qualifications. Where my enrolment relates to a qualification that requires work placement, I understand that I will be required to obtain a National Police Clearance and DHS screening check (Aged care, Vulnerable person, DIS and Working with Children) prior to commencing my placement. 

I authorise  HITsa to share the outcome of these clearance checks with the Work Placement Provider.

I understand that any disclosable court outcomes reported on the clearances may impact my ability to secure work placement and employment within the industry.

HITsa will charge a fee for all training conducted.  This fee may be paid by either the student, employer or a third party.  Fees must be paid in full by the first day of attendance unless a qualification course, and in this instance a fee payment plan must have been agreed upon, documented and signed off by all parties prior to the commencement of training.   

Fees are discussed prior to the commencement of training and policies and procedures are available at each site along with a copy on our website  www.hitsa.com.au

Each course or unit of training advertised will document all fees and charges relating to the course along with information of any eligibility criteria that applies for Government funded training.

Payment plans are available for courses over $500. 

Academic transcripts and/or parchments will not be issued if fees are outstanding.

Refunds are not granted automatically.  Once we receive your request in writing we will advise you and if eligible your refund will be processed within 14 days of receipt of written notification.  Please refer to our policy and procedure for further details.

If your Employer has funded your training and you leave your employment you may not be able to continue your study with HITsa.  Should the situation occur that you leave your employment and wish to continue your study to complete your qualification you will need to contact HITsa to discuss enrolment options.   

HITsa is required to charge a student co-contribution for Government funded training activity through the subsidised training list.  This co-contribution is payable on application  and is non-refundable should the student decide to withdraw or not complete the qualification.  Please refer to www.hitsa.com.au for full details on each offering.

I consent for HITsa Training and Employment (HITsa) to record my picture and voice on photographs, films and audio and videotapes, to edit these recordings at the discretion of HITsa Training and employment, and to incorporate these recordings into movie and sound films or audio and videotapes, webcast, streaming, broadcasts, (radio and television, including cable and satellite transmissions), programs or otherwise, to use in any manner of media whatsoever, including unrestricted use for purposes of publicity, advertising, and to use my name, likeness, or voice in connection therewith.

I hereby release HITsa from all claims for damages, for libel, slander, invasion of privacy or any other claim based on the use of the above-described materials.

I understand that the recordings are the exclusive property of HITsa and HITsa owns all rights to these recordings regardless of the form in which they are produced or used.

Further, I agree that I am not entitled to nor was I promised any compensation for the making of such recordings or their use.

I hereby authorise HITsa to use pictures and audio taken during my training as part of their marketing material and understand that these pictures may be used in brochures and/or Facebook or other social media.

I understand that I may withdraw this authorisation at any time, and this must be submitted to HITsa in writing.

The HITsa induction process is in two-parts, one of which you are completing now as you complete this application form. This confirms that HITsa has provided you with all relevant documentation prior to your decision to enrol so that you can make an informed choice.

I acknowledge that I have received and reviewed the policies and procedures viewable through the website prior to application.

Students will be provided with information specific to their training:

· Course information, fees and eligibility for fee free training (if applicable)

· Training plan and/or attendance requirements

· Employment pathways (if applicable)

Students will complete a site induction for each HITsa site attended on or before the first day training. The information supplied will include:

· Welcome and introduction to HITsa Training and Employment

· Structure for the day

· LLND requirements

I acknowledge I have been informed of the following information specific to the course during my enrolment and prior to the commencement of training.

HITsa uses closed circuit television at all sites for security purposes. This includes all areas of the venue and surrounding grounds. Footage may be viewed by Management at any time. Should footage show serious and/or wilful misconduct or failure to comply with reasonable direction, it will be used as evidence. Any footage containing illegal activity will be handed over to the relevant authorities.
